Cancellations
If you are unable to keep an
appointment, we ask that you kindly provide us with at least 48 hours
notice. This courtesy, on your part, will make it possible to give
your appointment to another patient.

Insurance
Our practice is a participating
provider with Medicare, Capital Blue Cross, Berkshire Health Plan
and Highmark Blue Shield. If your insurance is an HMO or PPO
you will be responsible for obtaining the necessary
referral. You will also be responsible for any annual
deductible or co-insurance. All co-pays are due at the time
of service.
